Student Health Update
If your child is diagnosed with a health condition (e.g. ADHD, asthma, an allergy), an updated Student Health Care Summary Form and any additional medical forms required must be completed by the enrolling parent/carer at the school office.
Allergies/Anaphylaxis
Allergic/Anaphylaxis specific forms are required to be completed and can be collected at the school office. It is extremely important that you inform your child’s teacher of any allergies your child has and the type of reaction they may experience.
Medication
Written permission must be provided for a staff member to administer medication at school. Parents must complete an Administration of Medication Form available from the School office. All medication brought to the school should be in the original container and must state the child’s name, type of medication, dosage and time medication is required.
Students taking medication will go to a designated area where the medication will be dispensed. All medication is to be signed in and signed out through the school office.
Medication required for short-term use must also follow the above procedures.
Unwell Students
Students who are unwell should not be sent to school. The parent/carer of a child who falls ill or is injured at school will be contacted and asked to take them home. Please arrange an emergency contact willing to look after your child when you are unavailable. In an emergency situation, a doctor or ambulance may be called.
Please ensure emergency contact details are always kept up to date. The enrolling person must complete a Student Update Form, available at the school office.
Immunisation
An AIR Immunisation History Statement is requested at the time of a student enrolment. Please refer to the WA Health – Immunisation Schedule to see if your child’s immunisation is up to date.

Head Lice Monitoring
Parents are advised to regularly check their child’s hair for live lice (nits) or eggs and to treat accordingly. Should a child be identified with live lice will be advised of the need for treatment and exclusion from school. The whole class is notified when there is an outbreak.
